Get generated sql query by calling: var query = ((System.Data.Objects.ObjectQuery)the_variable_that_you_return).ToTraceString(); and try to execute it directly against sql server. If it doesn't work, start to look for solution on the sql server side.

I'm not very familiar with phpunit, but you should run your tests one by one (not to initialize all of them at the same time)....or (if that is not possible), don't open your database connection on initialization, but inside the test. If that is not possible, too (thought I'm sure it is) then you really need many mysql connections opened at the same time, so there is nothing wrong to increase max_connections.

I would try closing connections earlier than teardown. You have one test method, close connection in every particular test.

Can you log and check if tear down is really executed? Also, how many connections do you open before tear down? It might happen that call to tear down is not "soon enough"...
