Like I said, there doesn't seem to be a firm justification for the weightings, but 4/6 approximately resembles 2/3rds, which resembles the amount of the area under a normal curve which lies within one standard deviation of the mean.

It is a communication problem. One thing I say to people is to never give a point estimate, always give a range. This getting a bit off the topic of how Scrum works though.

I disagree. Accuracy is not a binary condition, it's a fuzzy variable. An estimate can be more or less accurate and there are accepted metrics for measuring the degree of inaccuracy.

Well generally you use your system to track 1. what work to do, 2. what work is currently in-progress and 3. what work is yet to start. A good PM is basically looping around those three conditions to keep an eye on the overal progress of a project.
