The global :exclusions key simply adds the vector to each dependency. Your lein deps :tree should display [old-lib :exclusions [old-lib]] meaning that while fetching dependencies of old-lib any dependency on old-lib will be skipped. Check your lein classpath to see that old-lib is in fact on your classpath.
