I only put the parts I added to try to see if I missed a quote, a period, or some other unseen character. Of course the sql is being executed. I should point out that this form was created by a web developer who has php programmers employed - so for those who felt the need to bash the content I am just adding a few lines to the code provided to me. I realize there are many ways to do things, as witnessed by the answers provided and the corrections provided to those answers. I'm spending hours reading and trying to understand code, which to me is a foreign language. So telling me to find a better way to process the form is a useless reply. How about some constructive help as to what a better way may be?

Can you have both numbers and text in the value field, so that it returns both, or am I limited to either the number or the text.

Can the page be hacked if I am the only one who has access to it? This is a back end tool that only I can access, so I was not too concerned about hacks, unless I am in error and it is somehow accessible otherwise? This tool has been this way for 15 years with no issues as far as hacks.

Tried that, but then I lose all my custom formatting in the table head row - unless I am missing something else - I tried to use a class for the tablehead row to retain its formatting but can't seem to get that to work.

Still looking for a way to control the width of the table cells without having to add a class to each individual <td>. I have the code down to as bare bones as I think it can be, and the only problem is the cells are not expanding to fit the content of each one to fit a single line, which is the effect I need and can be accomplished by addressing each <td> but that seems quite cumbersome.

I'm still working on this - each time I make a cell a th it loses the formatting of the tr. Shouldn't the th inside the tr that is inside thead retain the formatting of the tr?

I know this is silly, but does CSS2 or CSS3 still use just style type="text/css"? Can you mix CSS2 or 3 in the same sheet as CSS1?

@Jukka, there have been attempts to make changes to content, but the code is so convoluted it usually messes up the look of the tables. Everything is thrown together with lots of span styles and inline styles - see a sample of the code

Using this method, I need to assign a class to every <td> element in my code, correct? Is there a way to assign the class to just the <th> element and have all the cells in that column have that styling? Is there a hierarchy between <th>, <tr>, <td> as to which code will take precedence over the other?

Today the video mysteriously appeared again on the site - but the skin with the video controls does not appear - just can't figure it out - hopefully get to speak to someone in support tomorrow.

I have read elsewhere that the issue could be server-related. I will assume that is the case since I uploaded the exact same video in the exact same manner on another site I run that is hosted on a different server and it plays just fine, as it did up until a few days ago on the server I now have problems with. I am waiting for a response from the host's support.
