Let me try the following design to see if it would work for you. You could create a standard ASP.NET MVC 4 site w/ the usual login page and wire that site up to a standard ASP.NET membership provider. Then, in the same site, you could add ASP.NET Web API controllers that act as your data service. In these controllers you could use the "[Authorize]" attribute to authenticate requests. Maybe this is what you're after?

comment is the notion of a Table required at all in Azure Table service?
Azure Table Storage is a very different design than a traditional RDBMS like SQL. You could design an app to just use a single table if you're not working w/ a bunch of relational data. Performance with tables becomes a problem if you can't do lookups based on the partition and row key. Julie has nice Azure Table Storage writeup you might be interested in reading.
