In my case, a wayward merge had obliterated the <value/> element, which is technically valid XML (no xml parse errors, etc.) but obviously not handled well by the RESX designer. thanks for the tip on "halving"...I was able to do it with an xml comment, which worked pretty well: <!-- ... -->

@DeanK. Interesting. I'd have to dig a bit to see if that was a ClickOnce caveat only. Is there anything in that config file whose absence might just be causing a silent crash? Connection String? etc?

Out of curiosity, what is the reason for a StartNew (sub task) inside the predicated condition? couldn't all relevant parameters (scheduler, etc.) be passed directly to the outer ContinueWith call?

The piece I was missing was the "construction of the DOM blocks until the external file is loaded" part (I haven't done much js work, so i was assuming the external file had to be in the <head/> and missed that i could still have the <script/> tag inline). Thanks for explaining not only how to do this, but also why it will be a suitable solution. Much appreciated.

I understand that this is the only allowed javascript mechanism for the new manifest_version, and i understand the underlying issue (the answer to the "duplicate" question). My question is not what is wrong with the old way...but rather what is the (new and recommended) way to use an external javascript file and never miss a <embed/> load event for the nacl module?
