You might want to consider specifying the encoding parameter like ImageFont.truetype(font_path,font_size,encoding="big5").

Sorry @MikeHoneychurch . I didn't get time to give a concrete code in my original answer, and I don't have a Mac at hand so was only able to verify the code on Win. I thought Mathematica is platform independent, so maybe it's not true for some interface related functions? (not sure about it..)

@Szabolcs Sorry for a hurry answer... and thank you a lot for your code :) I can not refer myself as familiar with the FrontEnd, but I did occasionally encounter the Chinese font problem a few days ago and did have strong interest on the FrontEnd programming.

comment Getting the Mathematica front end to obey the FontFamily option
and I found that in the UnicodeLanguageFontMapping.tr file, many common characters for both Japanese and Simplified Chinese are placed only in the # Japanese section. I tried to move those ones belonging to CJK Unified Ideographs and CJK Unified Ideographs Extension A to the # Chinese simplified section, with changing the encode from ShiftJIS to CP936, and found those characters are automatically recognized as Chinese characters and then assigned with DefaultChineseSimplifiedFont/DefaultMonoChineseSimplifiedFont now.
