I am afraid not. M.t seems to be closed if you want to write [> `A | M.t]. BTW, I think you need not to define M.t as opened, since make an opened poly variant type from a closed one is pretty easy like [> M.t].

This is not a problem of circularity. You cannot write an expression which may cause recursive computation in the rhs of let rec: see stackoverflow.com/questions/19859953/…

Please note that "implicit type variables" I used is not an official OCaml word. In OCaml everything instantiable must have type variables, including open polyvariant types [> `A | `B] and open object types <m:int; ..>. But you can omit writing them if you do not use them elsewhere. They are actually short ways to write type variables with constraints: 'a constraint 'a = [> `A | `B] and 'a constraint as 'a = <m:t; ..>. They are instantiable only to subtypes of their constraints. (You can also write equivalent like [> `A | `B] as 'a and <m:int; ..> as 'a.)

It's ok. My point is that if you want to use state monad, you can use it in OCaml too, and you do not need to use ref at all. I agree with @ivg about the use of state monad in OCaml: I use it sometimes but so rare that I do not feel do notation.

If you want to prevent one unique_id function from being misused more than one place, define a higher order function like with_unique_id : ((unit -> int) -> 'a) -> 'a whose argument takes a fresh function made by make_unique_id. BTW, if you want the pure solution of the state monad, you should keep things pure. Having a reference in the monadic state is very confusing.
