This turned out to be the solution in my case too. PHP was unable to properly detect the charset of the incoming csv file and just started mangling things without warning. Re-encoding lines worked perfectly.

When adding a new column, sixth option across is "Attributes". Binary, Unsigned, Unsigned zerofill, and on update current timestamp. Setting Unsigned here will give you an unsigned int column.

Searching a list of strings will be orders of magnitude slower than an re.search over the text block. Combining the regex if it is simple strings is faster still.

If you can't expect people to have memcache or apc installed, then you probably can't expect them to have write access via php either. Lots of shared servers don't give php write access.

FYI the numbers don't map directly. I've just come across an address that gives an accuracy of 6 with V2 and GEOMETRIC_CENTER with V3. Annoyingly maps itself can give a rooftop match.

Easier than using the full absolute path is the use dirname to let php figure it out. geoip_open(dirname(FILE).'"/geoip/GeoIP.dat",GEOIP_STANDARD);

Since the above answer is the best, I'll add here. The problem package is libmysqlclient15off which is a holdover from upgrading from an older release. Few of the older packages weren't upgraded or were dropped leaving it around on some hosts. I my case an old mydns install still depended on the older version.

Had forgotten about this. It wasn't a duplicate. Code was actually not getting a remoteip set as I said. Issue was eventually tied back to the script being called by an automated script. PHP doesn't set the IP variable not called through a webserver.

I work on a number of integration systems that require storing passwords in a way that means the system can automatically get back the plaintext version. Honestly it never feels good doing it and usually we do manage to get a completely separate account setup unique to the integration, but in the cases it can't, you can not simply just say no, especially for features customers want. For these we generally use GPG with keys not coded in the application or db.

In some cases on shared hosting servers, the DB engine/disk might have space left but your DB quote could have been exceeded. I've seen db quotes done via user quotas which has caused similar errors.
