For ASP.NET you use WIF. What version of VS are you using? e.g. for VS 2012 you would use the "Identity and Access Tool". This produces client metadata which you the import into TFIM. There is nothing specific you do to IIS itself.

ADFS works with AD on premise. However, it can integrate with Azure Active Directory in the cloud. AAD also supports both WS-Fed and SAML. There are others e.g. Ping, OpenAM that support both.

Not really - it's a custom attribute store which sits alongside ADFS - not an WIF extension. Then you use the custom attribute store as the source of the claim rather than AD.

Yes - so it looks like you've configured simpleSAMLphp to expect transient. But the rule above doesn't pass NameID - just email and UPN. You still need the transform.
