A sed solution:

sed -e ':a;$!{N;ba};:p' \
 -e 's/^\([^|\n]*|\)\([^\n]*\)\(.*\n\)\1\([^|\n]*\)/\1\2,\4\3/' \
 -e 'tp;s/\n*\n/\n/;P;D;bp' filename
