The relevant steps of macro expansion are (per C 2011 [n1570] 126.96.36.199 and C++ 1998 16.3.1):
- Process tokens that are preceded by
- Apply macro replacement to each argument.
- Replace each parameter with the corresponding result of the above macro replacement.
- Rescan for more macros.
xstr(foo), we have:
- The replacement text,

str(foo) is that step 2, which would replace
4, comes after step 1, which changes the argument to a string. In step 1,

This is why a helper macro is used. It allows us to get a step 2 performed, then use another macro to perform step 1.
