You need to add a fields or exclude attribute to the form class

class CategoryForm(forms.ModelForm):
    class Meta:
        model = Category
        fields = ['name']

Seems like you don't have a model named Category in that case you should inherit from forms.Form

class CategoryForm(forms.Form):
    name = forms.CharField(max_length=128, help_text="Please enter the category name.")
