I have a 32x32x3 (height, width, depth) image that I am trying to rotate around the z axis by 45 degrees in sitk. It appears however that the z/depth axis I am getting sitk to rotate around is at an angle. How would I be able to rotate the image such that if I look at one slice of my image, I will see the slice rotated from the center by 45 degrees?

Below is my code, and below the code are the images (first one is original image, second is failed attempt at rotation). Additionally, these are public images, not confidential data.

Based on the information provided here I have a suspicion of what is going on. I believe your MRI scan has a non unit direction cosine matrix. You can confirm this with:

print(img.GetDirection())

The output is in row major order. When you do:

img_arr = sitk.GetArrayFromImage(img)[0]

You are assuming that the direction cosine matrix is the identity. Thus when you grab a slice perpendicular to the third axis it is perpendicular to the z axis, which it is not (it may be close).

To rotate around the axis that is perpendicular to the axial image plane you need to take the third column of the direction cosine matrix as your rotation axis and you know your angle, together they define a rotation matrix (see here for details).

You can then do:

np_rot_mat = compute_rotation_matrix_from_axis_angle()
euler_transform.SetMatrix(np_rot_mat.flatten().tolist())
