What are the equivalent of these functions in gtk2Hs when migrating from gtk2 to gtk3
I'm not 100% sure, but I would start by checking these:
menuItemActivated
draw
buttonActivated
destroyEvent
The on prefixes have been done away with in favor of a single function named on.

user pointer in g_signal_connect is not passed correctly
My main suspect is that GdkEventButton event should have been GdkEventButton *event
It is very rare that you pass full structures as parameters in C (and in Gtk)

Draw a GtkImage with Cairo
You are drawing to the cairo surface, but "nothing knows", so no redraw is triggered.
A random idea to trigger a redraw would be to call gtk_image_clear() and then gtk_image_set_from_surface(...
